The risk of cancer development in the first year after diagnosis of prostate cancer has not been well characterized. We identified patients with localized prostate cancer who underwent a needle biopsy and then had an ipsilateral transurethral resection of the prostate (TURP) within 1 year. We identified those patients with low-risk disease, defined as prebiopsy prostate-specific antigen ≤ 10 ng/ml, Gleason score ≤ 6, and clinical stage T1c or ≤ pT2a, and assessed the development of advanced cancer and treatment strategies over a minimum 5-year follow-up. Among 1096 men who underwent a needle biopsy and then TURP, 258 men had low-risk disease. The risk of prostate cancer progression in the first year after diagnosis was low and was limited to a TURP that was converted to radical prostatectomy or radiation therapy. Overall, 18% of men with low-risk disease were diagnosed with advanced cancer, and 11% required treatment for prostate cancer. The risk of treatment in the first year after a needle biopsy was high, and was not influenced by the use of extended biopsy schemes. In conclusion, cancer progression in the first year after a needle biopsy was limited to cases with low-risk disease. In those cases, a TURP for patients with prebiopsy prostate-specific antigen > 10 ng/ml, Gleason score > 6, and clinical stage T2c or > pT2a was uncommon, and associated with the development of advanced cancer.
